Bluestem PACE — Program of All-Inclusive Care for the Elderly — partners with older adults and their families so seniors can remain safe and independent in their home for as long as possible. This is achieved by Bluestem PACE providing highly coordinated, integrated medical care to help participants and families achieve participant goals.
